The method
Step 01
We agree what the product must do, what it will not do yet, and what the first release contains. You get that in writing before any code exists.
Step 02
Work ships in short reviewable pieces. You see it running early, so you can change direction while changing direction is still cheap.

We handle release, store submission, and the first weeks in production. Then we keep it maintained on terms we agree up front.

Questions
It depends on what it does, so we scope it before quoting. Small apps start in the low thousands. You get a number and what is inside it before you commit.
A first release is usually weeks, not months, because we cut the first version down to what actually has to exist. The plan says the date.
That is a normal starting point. The scoping step exists to turn an idea into a build plan, and it works as a small paid piece on its own if you want to stop there.
You do, in full, including the repositories and the store listings. We hand over everything at the end.
Yes. Everything runs over email and calls, and the working notes are written down so time zones do not cost you anything.
